To proceed with services, we require specific documentation to comply with clinical, insurance, and regulatory standards. Below is a list of documents that are generally required:
Referrals can be initiated by parents, caregivers, physicians, schools, or other service providers. To start, click one of the links above: ONLNE REFERRAL FORM or CONTACT INTAKE TEAM. The online form will send you to our Central Reach online form. There you be require to fill out all the required information. Please be prepared with the child's prescription for Applied Behavior Analysis (provided by psychologist or pediatrician), the child's Diagnostic Assessment that notes Autism (provided by psychologist or pediatrician), and a copy of your insurance card. If you need support with a referral, please contact our intake team.
Once we receive the initial referral, our intake team will schedule a consultation with the family to understand the individual's unique needs and goals. During the meeting we will discuss our services, our team, and our history. This consultation helps us determine the appropriate level of service (clinic, in-home, telehealth, school-based) and allows families to ask questions about our programs.
Our billing team will verify insurance benefits and obtain any necessary authorizations for ABA therapy or other behavioral services. This process can take a few days to a few weeks, depending on the insurance provider. We keep families informed throughout this step and work to expedite the process as much as possible.
Once all documentation is received, insurance is verified, and authorization is approved, our clinical team will schedule the initial assessment and begin creating an individualized treatment plan. From there, we’ll match the client with a therapist based on their specific needs, and services will begin as scheduled.
