To proceed with services, we require specific documentation to comply with clinical, insurance, and regulatory standards. Below is a list of documents that are generally required:

1. Prescription Letter: A completed prescription letter from a healthcare provider or referring organization detailing the reason for the referral and any relevant background information.  This must include the prescription for Applied Behavior Analysis.
2. Diagnosis Documentation: A copy of the client’s diagnostic report confirming an autism spectrum disorder or other relevant diagnosis, typically from a licensed psychologist or pediatrician.
3. Insurance Information: Proof of insurance, including a copy of the insurance card (front and back) and any authorization forms required by the insurance provider.
4. Release of Information Form: A signed consent form authorizing AAI to communicate with relevant parties (such as school districts, other healthcare providers, or insurance companies) to ensure comprehensive care.  We will provide this form during the onboarding process.
5. Medical and Developmental History: If available, a copy of relevant medical records, documents from school services (e.g., IEP, 504 Plan), developmental history,  and any previous evaluations or assessments (e.g., ABA services) related to autism or developmental delays.

Referrals can be initiated by parents, caregivers, physicians, schools, or other service providers. To start, click one of the links above: ONLNE REFERRAL FORM or CONTACT INTAKE TEAM.  The online form will send you to our Central Reach online form.  There you be require to fill out all the required information.   Please be prepared with the child's prescription for Applied Behavior Analysis (provided by psychologist or pediatrician), the child's Diagnostic Assessment that notes Autism (provided by psychologist or pediatrician), and a copy of your insurance card.  If you need support with a referral, please contact our intake team.

Once we receive the initial referral, our intake team will schedule a consultation with the family to understand the individual's unique needs and goals. During the meeting we will discuss our services, our team, and our history.  This consultation helps us determine the appropriate level of service (clinic, in-home, telehealth, school-based) and allows families to ask questions about our programs.

Our billing team will verify insurance benefits and obtain any necessary authorizations for ABA therapy or other behavioral services. This process can take a few days to a few weeks, depending on the insurance provider. We keep families informed throughout this step and work to expedite the process as much as possible.
